He recalls working recently with a client
where employees said their job specs were outdated and no longer
reflected their actual responsibilities. In a couple of cases, he
adds, they didn’t even exist.

Regarding workflow documents, Dewsnap says
what is actually being done often won’t match what is written down.
A new system or process step will have been introduced, or
something is no longer done in that way, or employees will even do
everything differently, outside the system and without process
documentation.
